Social Care Network (SCN)

Helping eligible Medicaid members connect with community resources for food, housing, transportation, and care navigation.

Social Care Network (SCN)

Support that matters, where daily life happens.

Health is shaped by everyday life. Whether there is food in the kitchen, whether the heat is on, whether there is a ride to an appointment, whether anyone can help make sense of the paperwork: these things decide how well treatment can work.

The Social Care Network is designed to help eligible Medicaid members address health-related social needs and connect with trusted services in their community. Our team screens, reviews eligibility against applicable program requirements, and then helps with the navigation itself.

A screening is a conversation, not an application for care, and completing one does not guarantee eligibility or services. Eligibility depends on Medicaid enrollment, screening results, program requirements, service availability, and applicable New York State guidelines. We will always tell you honestly where you stand.
