Support that matters

Social Care Network

Because health is shaped by everyday life. The Social Care Network is designed to help eligible Medicaid members address health-related social needs and connect with trusted services in their community.

What the network can help with

Support with the things that make treatment possible

Whether there is food in the kitchen, whether the heat is on, whether there is a ride to an appointment - these decide how well anyone can focus on their health.

Food & Nutrition

Access to eligible food, pantry, nutrition, and health-focused support services.

Housing Support

Navigation to qualifying housing-related resources that can help improve stability and wellbeing.

Transportation

Help identifying transportation and community resources when available and appropriate.

Care Navigation

A simpler way to understand available services and connect with the right support.

Who may be eligible

Eligibility is based on more than one thing

Eligibility is based on Medicaid enrollment, screening results, health-related social needs, and applicable Social Care Network program requirements.

  • Individuals experiencing food insecurity or difficulty accessing nutritious food
  • People with certain chronic or behavioral health conditions
  • Pregnant or postpartum individuals with qualifying needs
  • Individuals facing qualifying housing or other health-related social challenges

How it works

A simple path to the right support

Choose a step to see what happens at each stage. Nothing is decided for you, and you can stop at any point.

Step 01

Complete a Screening

Tell us a little about your current needs and circumstances. A screening is a conversation, not an application for care, and you only share what you are comfortable putting in a general message.

Participation and services are subject to Medicaid eligibility, Social Care Network program requirements, screening results, service availability, and applicable New York State guidelines. Completion of a screening does not guarantee eligibility or receipt of services.

Start here

Not sure if you qualify? We can help.

Complete a short screening to see whether you or a family member may be eligible for Social Care Network services. Our team reviews every screening personally.

Submitting a screening does not guarantee approval or services. Eligibility depends on Medicaid enrollment, screening results, program requirements, service availability, and applicable New York State guidelines.

If you would rather talk it through first, call 914-352-6116 and ask for the SCN team.

What kind of support are you looking for?

Choose as many as apply. If you are not sure, choose the last option and we will talk it through.

Please do not include confidential or private information about your health condition in this form. Our team will arrange a secure way to discuss clinical details with you directly.

You are in the right place

Help is available. Taking the next step is simple.

Send a request, or call and speak to a person. We will help you find the right clinician, explain what your insurance covers, and answer the questions you have not thought to ask yet.
